The Occupational Safety and Health Administration (OSHA) requires certain establishments to report information from their injury and illnesses entered on their previous calendar year’s OSHA:
- Form 300A Summary of Work-related Injuries and Illnesses,
- Form 300 Log of Work-related Injuries and Illnesses,
- and 301 Injury and Illness Incident Report
Covered establishments must electronically submit these documents by March 2 of the following calendar year.

OSHA provides a secure website, the Injury Tracking Application (ITA), where you can manually enter your data to the ITA via their web form, upload a CSV file to the ITA, or transmit data electronically via an API (application programming interface).
